If you are:
- an active member wishing to apply for disability retirement
- a member who has received FPPA Board approval for a TAXABLE (off-duty) disability retirement application and now wish to receive the funds from your SRA (separate retirement account)
- a member who has received FPPA Board approval for a NON-TAXABLE (on-duty) disability retirement application and now wish to receive the funds from your SRA (separate retirement account)
- a DISABILITY RETIREE applying for a change in disability retirement status
- a SPOUSE or CHILD of a member who has died in active service
We have put together all of the forms necessary to submit to FPPA. The forms have been bundled together along with easy to follow instructions as well as a check-list to aid in completing the packet.

- Disability Retirement Application
This packet is for active members who are applying for disability retirement.
- Distribution of an SRA - for a TAXABLE (off duty) disability or survivor benefits
This packet is for members who have received FPPA Board approval for their taxable (off duty) disability retirement application or survivor benefits, and now wish to receive the funds in the SRA (separate retirement account).
- Distribution of an SRA - for a NON-TAXABLE (on duty) disability or survivor benefits
This packet is for members who have received FPPA Board approval for their non-taxable (on duty) disability retirement application or survivor benefits, and now wish to receive the funds in the SRA (separate retirement account).
- Application for Change in Disability Status
This packet is for disability retirees who are applying for a change in disability status.
- Application for Survivor Benefits
This packet is for a spouse or child of a member who has died while in active service.
