| Statewide Death & Disability Plan Forms If you are: - an active member wishing to apply for disability retirement - a member who has received FPPA Board approval for a TAXABLE (off-duty) disability retirement application and now wish to receive the funds from your SRA (separate retirement account) - a member who has received FPPA Board approval for a NON-TAXABLE (on-duty) disability retirement application and now wish to receive the funds from your SRA (separate retirement account) - a DISABILITY RETIREE applying for a change in disability retirement status - a SPOUSE or CHILD of a member who has died in active service We have put together all of the forms necessary to submit to FPPA. The forms have been bundled together along with easy to follow instructions as well as a check-list to aid in completing the packet.
