Retirement / Survivor Paperwork
- Signed and completed paperwork must be received by the 10th of the month in order to be processed that month. If received after the 10th, the paperwork will be processed the following month and will include retroactive benefits if applicable. Please note that all forms require a wet/physical signature. FPPA does not accept electronic signatures.
- The last day of active service and the retirement date may not be the same day. The retirement date may be the following day after the last day of active service as long as they are eligible at that time to begin their retirement benefit.
- If you are unable to locate a volunteer member who is now eligible to begin receiving their monthly benefit, contact FPPA for assistance in locating the member.
- A volunteer that is eligible may begin receiving their monthly pension benefit on their 50th birthday. The Retirement date does not need to wait to begin until the 1st of the following month – the retirement date can be the member’s 50th birthday
- You can now submit the new retiree/survivor information and upload the member forms directly through the Employer Portal via a secure email button. There is no need to submit a retirement/survivor application as you can now enter the data into the Employer Portal by clicking “Add a New Retiree”.
Common Mistakes / Tips for Success
- Forgetting to click the “Submit Changes” button when making changes through the Employer Portal. After entering your changes, click "Save" then select "Return to List," lastly click "Submit Changes." Otherwise, your changes will not be received by FPPA.
- Incomplete forms that are missing requested information.
- Using old forms - the most current form(s) are available on the Employer Portal or FPPA’s website.
- If a volunteer passes away prior to age 50 and has enough service credits to have begun receiving a pension benefit when the member would have turned 50, their spouse will now be eligible to begin receiving a survivor benefit, if applicable, on the date the volunteer would have turned 50.
- All volunteer departments must pay a one-time funeral benefit for an active or retired volunteer firefighter who has passed away. This benefit is payable to the spouse, relative or whomever is handling the funeral expenses for the deceased member. This is per Colorado Revised Statute § 31-30-1129. You can direct FPPA to pay a funeral benefit via the Employer Portal.
- Please do not send member forms (Electronic Funds Transfer Form, Form W-4P, Form W-9 Request for Taxpayer Identification Number and Certification) via regular email as this may compromise the member’s confidential information. Please be sure to use the link on the Employer Portal to send these securely.
